How the market is structured
The sector splits sharply. Around 7,500 pharmacies operate under traditional 503A parameters, compounding patient-specific prescriptions at modest scale. Fewer than a hundred registered 503B outsourcing facilities compound without patient-specific prescriptions and carry something close to manufacturing obligations. GLP-1 demand and telehealth fulfillment have pulled significant volume through both.
Regulation is the defining feature, not a side condition. State boards, DEA scheduling, USP standards and FDA registration all bear on what a facility may make, for whom, and in which states — and the rules differ by state. Software that treats compliance as a reporting feature rather than an enforcement layer is not usable here.
Where the opportunity is
Serialisation is the wedge. If every dispensed unit carries an identifier that links patient, order, finished lot, batch, raw lots and shipment, then a recall becomes a query rather than an investigation, and pack verification becomes a scan rather than a hope. Few systems serving this segment do it end to end.
Because the facility count is small and the switching cost is high, this is a land-and-expand market rather than a volume one. One facility proven, then the multi-site network around it. Directory and RFQ demand provide the introduction; the operating system provides the reason to stay.
